If you are thinking about traveling with your pet to a foreign country, you will need an international health certificate for your animal. Depending on the country, they may require:

At Touhy Animal Hospital, we are pleased to offer international health certificates for your pets. Please have all your travel information prepared ahead of time, including destination, length of stay, how you plan to board your pet, etc. For new clients, an exam is required to establish care prior to the international health exam. This is to review the travel plans and to assess the pet’s health. Please bring all necessary documentation and health records.

Please take a look at the following requirements 30 days before travel and submit the following PRIOR to your exam appointment:

Rabies certificate, (if not previously administered at our location please call your previous veterinarian before your appointment)
Proof of International Microchip with date of implantation
Completed travel form sent from our hospital, e-mail forms to touhyanimals@gmail.com or text us an image to (847) 698-3123.

Please note the following:

Signed & sealed travel certificates are only valid for 10 days from issue date
Microchip must be done prior to Rabies vaccine
Patient receiving a rabies vaccine must do so 21 days before travel
Check with your airline for requirements and visit: https://www.aphis.usda.gov/aphis/pet-travel and select the country of destination for up to date information as travel requirements are being updated frequently.
Post final exam, allow 5 business days for processing completed certificates, we will contact you as soon as we receive it.

*It is critical that we are provided with all the information above in order to comply with the travel requirements and highly recommend that you visit the link above prior to your visit and 30 days before travel to ensure that all requirements are met*

Please note: A cancellation made with less than a 24 hour notice significantly limits our ability to make the appointment available for another patient in need. Appointment cancellations with less than 24hrs notice will incur a $42.00 charge.
